This case study is prepared for my exam question one which worth 30 marks. The minimum word count is 300.
We can choose our own case study. EXCEPT (UK flash flooding and the Pitt Review, and the Cumbria flash flooding of 2009, Herald of Free Enterprise at Zeebrugge, Belgium
(1987) or M/A Estonia ferry in Baltic sea
(1994), and how RORO sinkings changed regulations for safety on sea, I-35W road bridge collapse, over the Mississippi river at
Minneapolis in USA, in 2007, Japanese 9.0 earthquake and the Fukushima nuclear plant
disaster, 2011, Concorde crash at Paris on 25 July 2000 and foreign object damage.) We have done these case study before so we can not use these in exam.
The question 1 is:
For the case study of your choice write an article for the lay-person reader covering the
following three learning outcomes:
• Show that lessons learnt from the incident or disaster can help to prevent similar
failures from happening again.
• Show that the root cause(s) to the incident or disaster is/are linked to the
management of safety by way of one of the three topics of structural reliability,
risk assessment or limit state design.
• Introduce the types of organisation and/or independent body who review and
monitor matters relating to the safety of the industrial sector.
