Format
• Type double-spaced
• Font: Times New Roman, 12 point, black ink
• Margins = 1 inch
• Staple to secure pages permanently; no plastic covers, binders or folders
• Use a header to state your name and assignment name; number pages; the first page of text is #1, etc.
• Write in narrative form in full sentences using paragraphs
• Use headings to delineate the sections of the paper
• Reference KEEPS Claims and explain the connection to your material

Part II. Functional Behavioral Assessment
• Description of the Dilemma (1.5 pages)
Describe the student’s challenging/problem behavior(s) and the nature of the problem.
Consider the dilemma from multiple angles. Draw on appropriate models of behavior and course readings/discussions to guide your description of the dilemma.
• Data Collection ( 2 pages + Appendix/Completed Data Collection Forms)
Collect data on the behavior(s) for 3-5 days. The data collection methods should offer multiple perspectives on the student (i.e., a variety of settings, curricular contexts, interactions). There should be at least 3 different forms of data collected (e.g., ABC analysis; anecdotal notes; interviews). Explain the rationale for selecting each method, describe the process used for implementing the method, and pros/cons of each method. Attach all completed data collection forms to the paper. Documentation of interviews should include a question/answer format I will upload the format to complete it and I will upload example for it . Completed data collection forms are an appendix and should not be considered in the pages required for Part II.
